Rental Costume Ordering Instructions & Forms

Costume America offers convenient ways to order rental costumes for your next production. You can call us at (631) 414-7464 to reach our friendly staff. You can also fax your order to (631) 414-7466, or email to info@costumeamerica.com.

In order to reserve your costumes and lock in your dates, we require the rental order form, a copy of the signed rental agreement, and a list of costumes filled out on our measurement form. If your show date is within one month away, we require definitive measurements upon receipt of rental agreement. If your show is within 1-3 months away, measurements are required as soon as possible. If your show dates are 3-6 months away you can send measurements after reserving your costumes. Once we receive your order forms and your 25% deposit the costumes are held for you until your show dates.
